CareGap

Improve Patient Care. Manage Population Health. Engage your Community.

Early disease awareness improves lives

When patients focus on improving their health and avoiding return visits to the hospital, everybody along the healthcare continuum feels better. That’s why we created CareGap. CareGap is a powerful and unique workflow and patient engagement interface for clinicians. Designed to prevent “avoidance admissions,” readmissions and CMS penalties, CareGap improves communication between a hospital and discharge facilities – empowering better care transitions with fewer complications. CareGap also provides structure for and reporting on interactions with patients following hospital discharges. The end result is higher patient satisfaction and health coupled with lower costs of care and a more organized care team. The CareGap programs cover an array of conditions and procedures, including structural heart, diabetes, CHF, COPD, and AMI.

How CareGap Works

CareGap is accessed through HealthAware’s HIPPA-compliant website, CIMS (Clinical Information Management System). Each CareGap user is issued a username and password to access the site, where they can then enter data, generate reports, check for dashboard updates, and more. CareGap can be easily accessed from any web-enabled device at any time, and there are no requirements to purchase hardware or install software in order to start using CareGap. Yes, it’s really that easy. By design.

Implementing CareGap

CareGap is as easy to implement as it is to use. HealthAware’s experienced Service Delivery team works with our partners to create customized versions of our CareGap programs that are tailored to meet the specific needs of each client – based on available resources, team members, and existing structure. CareGap offers 24/7 access via the web and can be easily accessed from any web-enabled device. Remember, all you need to access CareGap is your username and password. It sounds easy, because it is.

– Jeff Blankenship – CFO, West Tennessee Healthcare

“When we implemented HealthAware’s Kare-N™ blood program to help reduce our no-show rates for member appointments, we immediately saw the huge impact by reducing our No Show Sates by 33%, above and beyond existing text reminders, resulting in a more profitable operation.”

– Jamie  – VP of Operations, The Blood Centers

“Our CHF program is successful (above CMS targets) but we were in need of a solution to provide structure, analytics and reporting so that we could continue to focus on our patients’ care. Nothing was the right fit until we found HealthAware™. The real-time tracking and reports provided through HealthAware have dramatically improved the accessibility and clarity of our readmission and mortality analyses. We now have previously unavailable insights into our patient data that give us the knowledge we need to make valuable procedural and operational changes for the betterment of our patients’ health and our health system. HealthAware has become an invaluable part of our care plan including transitions and post-acute care for CHF patients.”
