How CareGap Works

CareGap is accessed through HealthAware’s HIPPA-compliant website, CIMS (Clinical Information Management System). Each CareGap user is issued a username and password to access the site, where they can then enter data, generate reports, check for dashboard updates, and more. CareGap can be easily accessed from any web-enabled device at any time, and there are no requirements to purchase hardware or install software in order to start using CareGap. Yes, it’s really that easy. By design.

Implementing CareGap

CareGap is as easy to implement as it is to use. HealthAware®’s experienced Service Delivery team works with our partners to create customized versions of our CareGap programs that are tailored to meet the specific needs of each client – based on available resources, team members, and existing structure. CareGap offers 24/7 access via the web and can be easily accessed from any web-enabled device. Remember, all you need to access CareGap is your username and password. It sounds easy, because it is.

CareGap Reporting Tools

CareGap users can access, print and share reports designed for the patient, the discharge facility, physician communications, administrative review, and registry compliance.

All CareGap reports are customizable and can be modified for use in HEDIS compliance, 30-day mortality, readmission tracking, primary diagnosis analysis, continued care report, Patient Action Plans, deficiencies analysis, readmission risk analysis and various other applications.

CareGap also includes clinical dashboards, automated notifications, prompts for data collection and triggers for conducting follow-up at specific times.
